Join us for a two part Advance Care Planning (ACP) mini series aimed at supporting you in the ACP process. Whether you are working on your own ACP or that of a loved one, you are welcome here. If timing is a limiting factor and an asynchronous option works better for you, we can share the materials and guidance with you for you to work through in your own time.
What are your values? What are your wishes? What are all the intersecting pieces that will honour one's emotional, cultural, social, and spiritual needs? Led by Kristen Lew-Byrne of @growing.in.grief., each session will include education, reflection, Q & A, resources, and more! The hope is each participant will feel equipped in starting and ongoing ACP, while also passing along the learnings and takeaways to friends, family and loved ones.
