Join Growing in Grief and the Grief House for a digital Advance Care Planning (ACP) download. This digital download is intended to be self-initiated, and worked through at your own time and pace. We welcome those who are working on your own ACP or that of a loved one. 

This download includes a set of digital slides, as well as digital worksheets, centered around your values, your wishes, and all the intersecting pieces that will honour one's emotional, cultural, social and spiritual needs. The hope is each participant will feel equipped in starting and ongoing ACP, while also passing along the learnings and takeaways to friends, family and loved ones.
