What
This workshop, led by hospice nurse Gillian, provides a time and space to have important conversations about end of life wishes for yourself or someone dear to you. Advance care planning is spelling out your end-of life-wishes and making sure that the right people have that information, so that even if you are not able to speak for yourself, your wishes are carried out. This is important for everyone, regardless of age and health status, but is even more critical for folks with serious health issues or who are getting older. In this workshop, we will talk about POLST forms, Advance Directives, and other ways to document what kind of medical care you would want in different scenarios, and who you want making decisions for you if you are unable to speak for yourself. You will have the opportunity to fill out an advance directive form during the workshop if you wish to. If you know who you would choose to be your health care representative, bring them with you to the workshop so you can talk about your goals together!
