The Grief House and Growing In Grief have collaborated on a self-directed, What Are Your Wishes - Advance Care Planning offering. This self-directed ACP offering is intended to be an initial starting point. The offering includes a set of digital slides as well as digital worksheets. The hope is you begin reflecting on what is important to you, while talking about and writing your wishes. "Preparing for death is one of the most empowering things you can do (Chang, C)." It is never too early to start advance care planning, whether you are working on your own ACP or that of a loved one.
**Please be mindful as each Country, State/Provinces, requirements and laws may differ, when it comes to Advance Care Planning.
